Any ideas why the built-in preview would just stop working?

Try switching between Chrome/IE preview to see if it makes any difference. You can try to reinstall the editor.

Was able to get it working again.
I had added html files to the PHP document types so that I could get PHP syntax highlighting in ExpressionEngine HTML files that have PHP code. ExpressionEngine stores PHP code in HTML files when you create templates.
