Add ability to change Print Footer/Header

Add ability to change Print Footer/Header

I would like to be able to add the following to the header/footer when I print:
  • Filename
    Path
    Date printed
    Time printed
    Custom text
    Total number of pages
Bonus points for having different header/footer for the first page, odd pages, and even pages.
